Frimley Health and Care Integrated Care System (ICS) is a partnership of NHS and local government organisations working together to join up health and care services to improve the health and wellbeing of people in the communities we serve.

We are committed to creating healthier communities through reducing health inequalities for all of our residents and increasing healthy life expectancy for all of the people we serve.

We're supporting people earlier to help prevent ill health with conditions such as obesity, diabetes and mental illness, so they can be as active and independent as possible.

We're providing care and quality, as people are living longer, with more long-term conditions, requiring a wide variety of health and care services.

We're improving efficiency, as growing demand and funding challenges are creating a gap between resources and patient needs.

Find out how:

Currently more people are admitted to hospital than necessary, who then stay in hospital longer than they need to, because not enough services are provided in the community. Connecting local care and voluntary services together helps keep people healthy and supported at home for longer.

People may receive care in their own home or the town where they live, rather than having to travel to their GP surgery or a larger hospital. Patients may be treated by a physiotherapist, a counsellor, a paramedic or any of a wide range of appropriate professionals. Different organisations working together means people get the right care more quickly and easily.
